REQUIREMENTS FOR SUBMISSION

Requirements for new students from Grades 1 to 12 are the following:

• Original Report Card (F138) with Eligibility to transfer signed by the School Head

• Official Transcript of Records (F137) 

• Good Moral Certificate

• Copy of Vaccination Card

• Signed Consent Forms (available at the Registrar's Office)

• PSA Copy of Birth Certificate* Copy of Baptismal Certificate* (if Catholic)

• Copy of Parents’ Marriage Contract* (if married)

• 2 pieces of 2x2 ID Photos*

• Copy of Diploma** Copy of School ID (back to back)**

• ESC Certificate for Incoming Grade 11 students (if from Private Schools)**

The four requirements with an * are the only requirements for new Preschool students. The two items labelled with an ** are additional requirements for new Senior High School students.​

Additional requirements for Foreign Students:

• Copy of Passport showing bio-page, admission, and authorization to stay

• Household Register/Resident Register

• Level Placement Certificate from the Philippine Department of Education (DepEd)

• Special Student Permit (SSP)

• Alien Certificate of Registration (ACR)

Additional requirements needed for Special Education:

• Neurodevelopmental Evaluation Report from a Developmental Pediatrician (within 12 months)

• Therapy Report (e.g. speech therapy, occupational therapy, physiotherapy)

• Current Individualized Education Plan

On-campus registration could also be done from Monday to Friday, 8AM to 4PM. Saturdays could be scheduled by appointment.
